Firstly, let me introduce myself. I've been off the bike for a long while and am looking to ride again.
I've seen a 2005 R1150R with 65000km on the clock. I think the bike looks magnificent and from the few hundred reviews I've read on-line, it seems to be a good buy.
What specific issues should I be looking for with this bike with such high mileage?
And more generally, what should I look out for?

Not being smart, 65k is not high mileage, a well maintained 1150r feels very new at that age.
my personal experience (bike of simliar mileage)
you will benefit hugely from new or revalved shocks, front needs replacing, rear can be revalved, suggest ohlins
you may come across issues with wiring at the headstock - can be relieved by clipping zip ties if not already done
you may at some pit have issues with the final drive as for all 1150s, check for play in rear drive

You didn't say whether or not you have a maintenance book with the maintenance records in it, or the price asked for. The engine itself will go far beyond 65K, what you have to know is if certain things were done as routine maintenance, and what things have been done as replacement parts that already wore out...like bearings, o-rings, seals, drive shaft.
If it still has OEM shocks they are well worn for sure as suggested above. Clutch is another thing that may need replacing soon, depending on how it was ridden during those 65K miles.
The starter may be about to go, and you can have it rebuilt or just clean it and see how long it lasts. It should give you plenty of warning with noises.
So, there are many variables, which make it impossible to give a good answer, but depending on the price you could have a winner. It's a nice fun bike.
